Multidisciplinary : Patients seen by providers from two or more disciplines (e.g., nursing, psychology, nutrition, medical specialties, etc.), Consultative Service : One time, or additional upon request, visit to address survivorship issues, provide treatment summary, care plan and refer to subspecialties for further evaluation).

The Bon Secours St. Francis Survivorship Clinic is integrated with the Adolescent and Young Adult Oncology program and focuses on comprehensive evaluation of and recommendations for monitoring for persistent side effects and treatment related late effects of cancer therapy.
